Who we are

Pursuit Physical Therapy & Performance ("Pursuit", "we") is a physical therapy practice at 3b Condon Way, Hopedale, MA 01747. You can reach us at tj@pursuitptp.com.

What we collect

When you book an appointment through this website, we collect what you enter in the form: your name, email address, phone number, the service you are interested in, your general area of concern, what you would like to get back to, how and when you prefer to be contacted, your town and how you heard about us. When you pick a time, we also collect your booking details.

Along with the form, we record the page you booked from and any campaign details in the link you followed (such as utm_source or an ad click ID), so we know which pages and ads bring people to us.

Please do not include detailed medical information in the form. We take your health history at your first visit and handle it as part of your patient record.

How we use it

We use your information to reply to your request, schedule and remind you of appointments, answer your questions and improve our website. We do not sell your personal information.

Who helps us

Our booking form, calendar and contact records are provided by HighLevel (also known as LeadConnector), which stores this information on our behalf. Appointments sync with our Google calendar. These providers only use your information to provide their service to us. The booking form may set cookies needed for it to work.

Text messages

If you tick the box for appointment text messages, we may text you about your request and appointments, such as confirmations and reminders. If you tick the box for occasional texts, we may also send tips and offers. Consent is optional and is not a condition of booking or receiving care.

Message frequency varies. Message and data rates may apply. Reply STOP to unsubscribe at any time, or HELP for help. You can also contact us at the email address above.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text message opt-in data and consent are not shared with any third parties.

Email

If we email you, every non-appointment email includes a way to unsubscribe.

How long we keep it

We keep enquiry and booking records for as long as we need them to provide care and run the practice, and as required by law. Patient records are kept as required for health care providers in Massachusetts.

Your choices

You can ask us to see, correct or delete the information you shared through this website by emailing tj@pursuitptp.com.

This website is not intended for children under 13, and we do not knowingly collect their information online. Parents and guardians can book on behalf of a child.

Changes

If we change this policy, we will update it on this page with a new date.
